Have you received an AusVaxSafety vaccine safety survey through Vaxtracker? If so, please be involved, we want to hear from you. Your vaccination experience is important. The information that you provide through vaccine safety surveys helps to ensure the safety of all Australians.

Vaxtracker is primarily implemented in State Government vaccination facilities but is also implemented in some Aboriginal Medical Services, GP practices and pharmacies. Vaxtracker collects data across all Australian states and territories.

Anyone who receives a vaccination through a NSW Health staff clinic or NSW School Vaccination Program should receive the AusVaxSafety vaccine safety survey from Vaxtracker. However sometimes there is missing information in the data we receive from NSW eHealth (e.g. email address) which means you cannot be enrolled into Vaxtracker. Unfortunately, we cannot register participants manually.

In all other participating clinics in other states, to receive the vaccine safety surveys, you must register by scanning the QR code at your clinic and fill in a registration form at the time of your vaccination. We cannot register participants retrospectively.

If you have experienced a reaction following your immunisation and would like to report it, you can contact the Therapeutic Goods Administration (TGA). Please phone 1300 134 237 Monday-Sunday 8am-8pm or report online at Reporting adverse events | Therapeutic Goods Administration (TGA).

In Queensland, South Australia, the Australian Capital Territory, the Northern Territory, Western Australia and Tasmania to receive the vaccine safety surveys you must scan the QR code at your clinic after each dose.

As the Vaxtracker system is automatic, once your survey time has elapsed the survey cannot be reopened and we cannot send you a new link.

We are unable to assist with immunisation records, as we do not have access to the vaccination records system. You will need to visit the Services Australia website to find out how to access your digital records or call the Australian Immunisation Register (AIR) on 1800 653 809 to request a hard copy.

Vaxtracker is operated by a NSW Health team in the Hunter New England Local Health District. Surveys are sent through the Vaxtracker system via NSW Health on behalf of your State/Territory/Health service.

The security of your personal information and the information you provide in your surveys is very important to the team at Vaxtracker. All information you provide will be kept confidential, in accordance to the Health Records and Information Privacy Act 2002. The information you provide will be stored on password protected databases and your personal email address will be kept in a separate file to the health information you provide to us.
